ud. = usted

¿Tiene ud.? is very polite. Use this when speaking to people with respect and to people you don't know.

¿Tienes ? - if familiar.  Use this when speaking to someone you know and to children.

You could just say "¿Tiene?" or "¿Tienes?" and leave out the prounouns "ud." and "".

Sometimes you see, "¿ tienes? or ¿Tienes ?  Is there a difference?
No, It really doesn't matter if you put the "tú" or the "ud." before or after the "tienes/tiene" or even leave the "ud." or the "tú" out all together.  Either way is ok, because the intonation of your voice tells the listener that it's a question. 
       
When writing, the "question marks" indicate it's a question. I prefer to put the "tú" and the "ud." after the "tienes / tiene" when asking the question, and I put the "tú and the "ud." in front of the tienes / tiene when making a statement.
          
You do not put the "tú" or the "ud." after the "tienes / tiene" to make a statement. To make a statement, you put the pronoun in front of the conjugated verb "tienes / tiene".
                  
Example: (Statement)
Tú tienes  - You have....
Ud. tiene -  You have....
Tú tienes la pelota. - You have the ball.
Ud. tiene las llaves. - You have the keys.
          
Example: (Question)
¿Tienes tú? - The pronoun comes after the conjugated verb "tienes".
¿Tiene ud? - The pronoun comes after the conjugated verb "tiene"
          
Is there a difference between "¿ tienes?" and " tienes..."  ?
Yes, because one is the question and the other is a statement. 
            
Listen to the difference between "¿Tú tienes?" and "Tú tienes......"
   "¿Tú tienes? -  This is a question....
  "Tú tienes....."  This is a statement.....
